Covid-19 Prescreen

At the time of booking you declare that you do not have any signs or symptoms of Covid 19 including:

• fever, cough, difficulty breathing, nasal congestion, sore throat, headache

• dizziness, nausea, diarrhea, extreme fatigue

• loss of appetite, loss of sense of taste or smell

• flu-like symptoms

• you or a close contact have not travelled outside of Canada in the last 14 days and been told to self isolate

• you have had no exposure to someone who has tested positive for Covid 19 or is under investigation for Covid 19 in the last 14 days
